I sent the whole top in. I taped the cardboard into the plastic and sent both parts in. Probably only needed the cardboard part, but it said the "top" I think, so I sent the whole top. Better safe than sorry.</description>

If you don't mind me asking, what part of the 2004 box did you send in? On mine, there's a plastic "cap" with a barcode stuck on it. Inside the plastic cap is a cardboard insert that says it's the official proof of purchase. Do I send in the whole plastic top? Or just the cardboard insert?
